Private Coaching with Patrice

Coaching is peer-led, educational and motivational support based on the coach's personal experience and coaching training. Coaching is not medical, dental, orthodontic, psychological, therapeutic, nutritional or any other form of licensed professional advice, diagnosis or treatment.

Your coach is not a dentist, orthodontist, physician, therapist or other licensed healthcare provider, and no doctor–patient, dentist–patient or therapist–client relationship is created by Coaching.

Coaching does not replace care from a qualified professional. You should consult your dentist, orthodontist, physician or other qualified provider before starting, changing or stopping use of any Reviv product or any oral, health or wellness routine, and about any symptom, pain, discomfort or concern. Never disregard or delay seeking professional advice because of anything said during Coaching.

If you experience pain, injury, swelling, bleeding, loosening of teeth, jaw problems, breathing difficulty or any other adverse effect, stop using the product and seek professional care immediately. In an emergency, call your local emergency number.

You must be at least 18 years old (or the age of majority where you live) to purchase or attend Coaching. By purchasing, you confirm that you meet this requirement and that the information you provide is accurate and complete.

Individual experiences with the Reviv process vary widely and progress is often non-linear. We and your coach make no promise, representation or guarantee of any particular outcome, including any change in tooth position, jaw, facial structure, bite, comfort, appearance or health. Any experiences, examples or testimonials shared are personal and not a prediction of your results. You are solely responsible for your decisions and actions, including how and whether you use any product, during and after Coaching.

After checkout you will receive an email with a booking link. Sessions are approximately 30 minutes and held by video call.

All Sessions in a package must be booked and used within three (3) months of the purchase date. Unused Sessions expire after this period without refund or credit.

Before your first Session you may be asked to complete a short intake form. Please do not include information you are not comfortable sharing.

You are responsible for a stable internet connection, a working device and a private space for your Session. Time lost due to your technical issues or late arrival will not be added or refunded.

Coaching packages are personal to you and may not be transferred, resold or shared.

All sales are final. Coaching fees are non-refundable and non-exchangeable, in whole or in part, including for unused, partially used or expired Sessions, change of mind, dissatisfaction, or your decision to stop using Reviv products.

By purchasing, you agree not to initiate a chargeback or payment dispute for Sessions purchased in accordance with these Terms. We reserve the right to contest any chargeback and to suspend access to Coaching while a dispute is pending.

You may reschedule a Session free of charge with at least 24 hours' notice before the scheduled start time, using the booking link or by contacting us.

Sessions cancelled or rescheduled with less than 24 hours' notice, or missed without notice ("no-show"), including arriving more than 10 minutes late, are forfeited and count as used.

If your coach needs to cancel or reschedule, we will offer you a new time. If a Session cannot be rescheduled within the validity period, our sole obligation will be to provide a replacement Session with the same or another coach, or, at our sole discretion, a credit for that Session.

We may replace your coach, or modify, suspend or discontinue Coaching, at any time. This is a pilot program and its format may change.

You agree to treat your coach respectfully and lawfully. We may end any Session or terminate your access to Coaching, without refund, if you are abusive, harassing, discriminatory, threatening, or otherwise behave inappropriately, or if you breach these Terms. You may not record any Session without the prior written consent of both your coach and Reviv.

You understand that use of oral appliances and changes to oral or wellness habits carry inherent risks. You voluntarily choose to participate in Coaching and to act (or not act) on anything discussed, and you knowingly and expressly assume all risks arising from your participation in Coaching and your use of any product, whether known or unknown.

Some jurisdictions do not allow certain exclusions or limitations of liability, so some of the above may not apply to you. In such cases, our liability will be limited to the maximum extent permitted by law.

To the fullest extent permitted by law, you release, waive and forever discharge the Reviv Parties from any and all claims, demands, losses and liabilities of every kind, known or unknown, arising out of or relating to your participation in Coaching or your reliance on anything said during Coaching. If you are a California resident, you waive California Civil Code Section 1542, which states: "A general release does not extend to claims that the creditor or releasing party does not know or suspect to exist in his or her favor at the time of executing the release and that, if known by him or her, would have materially affected his or her settlement with the debtor or released party." You waive any similar law in any other jurisdiction.

You agree to defend, indemnify and hold harmless the Reviv Parties from and against any claims, damages, losses, liabilities, costs and expenses (including reasonable attorneys' fees) arising out of or relating to (a) your participation in Coaching, (b) your use or misuse of any product, (c) your breach of these Terms, or (d) your violation of any law or the rights of any third party.

Coaches may be independent contractors. Statements, opinions and experiences shared by a coach are their own and do not necessarily represent the views of Reviv, and do not constitute product claims, instructions or warranties by Reviv. Always follow the official product instructions and the advice of your qualified healthcare provider.

Information you provide in your intake form or during Sessions will be handled in accordance with our Privacy Policy. Your coach will make reasonable efforts to keep what you share confidential, but Coaching is not a privileged or legally protected relationship, and confidentiality may be broken where required by law or where there is a risk of harm to you or others. Video calls are conducted through third-party platforms, and we are not responsible for their security or availability.

Any materials provided in connection with Coaching are owned by Reviv or its licensors and are for your personal, non-commercial use only. You may not copy, distribute or publish them without our written permission.

Please read carefully – this affects your legal rights. Any dispute, claim or controversy arising out of or relating to Coaching or these Terms will be resolved by final and binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, rather than in court, except that either party may bring an individual claim in small claims court.

Class action waiver: You and Reviv agree that each may bring claims against the other only in an individual capacity, and not as a plaintiff or class member in any purported class, collective or representative proceeding. You waive any right to a jury trial.

Any claim must be brought within one (1) year after it arises, or it is permanently barred, to the extent permitted by law.

These Terms are governed by the laws of the State of Delaware, USA, without regard to its conflict-of-law rules. Subject to Section 16, you consent to the exclusive jurisdiction of the state and federal courts located in Delaware, USA

These Terms, together with our general Terms of Service and Privacy Policy, form the entire agreement between you and Reviv regarding Coaching. If these Terms conflict with our general Terms of Service, these Terms control as to Coaching.

If any provision is found unenforceable, it will be enforced to the maximum extent permissible and the remaining provisions will remain in full effect.

Our failure to enforce any provision is not a waiver of it. You may not assign these Terms; we may assign them without restriction.

We may update these Terms from time to time. The version in effect at the time of your purchase applies to that purchase.
